Distal femoral replacement is indicated where the distal femur cannot be reconstructed by fixation: oncologic resection of a primary bone tumour, failed revision arthroplasty with massive metaphyseal bone loss, or a comminuted periprosthetic fracture in poor bone stock. Planning requires full-length alignment films, CT for resection level, and templating of the resection length against the contralateral limb so that joint line and limb length are restored. Confirm extensor mechanism integrity and soft-tissue envelope before committing to a megaprosthesis.

Faculty-recorded distal femoral replacement demonstrating resection level planning, canal preparation, trial reduction and cemented megaprosthesis implantation, with extensor mechanism and soft-tissue management.
